We are seeking a dedicated LPN to join our Home Health team. The ideal candidate will provide compassionate care to patients in the comfort of their homes, ensuring their well-being and promoting their independence. *Duties* – Administer medications and treatments as prescribed – Monitor patient vital signs and health status – Educate patients and families on disease management and preventive care – Coordinate with healthcare providers to ensure continuity of care – Document patient progress and communicate effectively with the healthcare team.
